The purpose of this study was to test whether therapeutic guidelines for working with Transgender and Gender Diverse People (TGD) people were seen as effective and which aspects were most helpful. Thematic analyses revealed three main themes of affirming, effective therapy: (1) therapists understanding nuances between TGD identity and mental health, (2) therapists actively making an effort to mitigate clients’ fear of marginalizing therapy, and (3) therapists fostering pride in a client's TGD identity. These findings provide support for updated training and guidelines to assist therapists working with transgender and gender diverse (TGD) folx.

- [00:00:18.200]This study considered transgender and gender diverse people
- [00:00:21.240]or TGD people to be anyone whose gender identity
- [00:00:24.090]does not line up with their sex assigned at birth.
- [00:00:27.310]TGD people face various barriers
- [00:00:30.020]when seeking affirming psychotherapy.
- [00:00:32.180]This is for a variety of reasons
- [00:00:33.800]that will be discussed in the background.
- [00:00:36.210]There are currently minimal guidelines
- [00:00:37.760]for clinicians working with TGD people.
- [00:00:40.160]These guidelines were also written
- [00:00:41.410]based on the perspective of the clinicians,
- [00:00:43.270]rather than the TGD clients themselves.
- [00:00:46.310]The present study aimed to gain a better understanding
- [00:00:48.540]on what improvements are necessary
- [00:00:49.980]in order to create a more affirming
- [00:00:51.480]and effective therapeutic environment
- [00:00:53.350]through gathering data directly from TGD people.

- [00:00:58.210]have a long history of being subjected to marginalization
- [00:01:01.240]in societal, cultural, political,
- [00:01:03.520]and interpersonal settings.
- [00:01:05.720]This includes when seeking psychotherapy
- [00:01:07.530]and other medical help.
- [00:01:09.280]There are currently guidelines in place
- [00:01:10.880]to limit marginalizing behavior towards TGD people,
- [00:01:14.290]but some of these guidelines are ambiguous.
- [00:01:17.000]In the past,
- [00:01:17.833]most literature emphasizes the connection
- [00:01:19.420]between sexuality and mental health,
- [00:01:21.630]rather than gender identity and mental health.
- [00:01:24.500]There was very little research done
- [00:01:26.340]regarding TGD people's therapeutic experience
- [00:01:29.037]and all the literature regarding affirming psychotherapy
- [00:01:31.730]for TGD people has been written
- [00:01:33.410]from the therapist perspective.
- [00:01:35.820]The purpose of the current study
- [00:01:37.090]was to examine the nature of language and behavior
- [00:01:39.370]of therapists and the impact on TGD clients.
- [00:01:42.820]Understanding the relationship between therapist behavior
- [00:01:45.040]and client comfort can allow for improvement
- [00:01:47.090]in the therapist guidelines and trainings.
- [00:01:49.917]The hypothesis of the study was that the affirming videos
- [00:01:53.530]would receive the highest session ratings
- [00:01:55.550]with the marginalizing videos receiving the lowest ratings.
- [00:01:59.250]We also hypothesized that the neutral videos
- [00:02:01.410]would be perceived as more negative than positive.
- [00:02:04.490]Furthermore, the method of this study
- [00:02:06.220]was to ask participants to watch four successive recordings
- [00:02:09.020]of a mock therapy session.
- [00:02:11.130]There were three sets of videos
- [00:02:12.450]with varying therapist's behavior
- [00:02:14.010]that participants could be randomly assigned to,
- [00:02:16.610]affirming, neutral, or marginalizing.
- [00:02:19.620]Participants were recruited from the Central Great Plains,
- [00:02:21.910]which for the purposes of this study
- [00:02:23.570]consisted of the states of Colorado, Iowa,
- [00:02:27.290]Kansas, Missouri, Nebraska,
- [00:02:30.380]North Dakota, South Dakota, and Wyoming.
- [00:02:34.350]Participants also had to be of 19 years of age or older.
- [00:02:38.400]Participants were recruited by placing flyers
- [00:02:40.730]in popular TGD affirming places
- [00:02:42.728]and through listservs.
- [00:02:45.330]Participants were asked to rate their assigned videos
- [00:02:47.680]using the session rating scale,
- [00:02:50.000]and then participate in a semi-structured interview.
- [00:02:53.600]A quantitative analysis of the rating scores was conducted
- [00:02:56.460]and found that there was a significant difference
- [00:02:58.300]between ratings of affirming, neutral,
- [00:03:00.230]and marginalizing therapist's behavior.
- [00:03:02.900]Marginalizing had the lowest rating
- [00:03:04.530]with an average of 7.35.
- [00:03:06.760]Neutral was 26.05,
- [00:03:09.397]and affirming was the highest with 38.56.
- [00:03:13.360]The qualitative analysis was conducted
- [00:03:15.340]via thematic analysis of the results from the interviews
- [00:03:18.210]to determine three overarching themes.
- [00:03:20.860]These themes for therapist's understanding the nuances
- [00:03:23.100]between TGD identity and mental health,
- [00:03:25.580]therapists, actively making an effort
- [00:03:27.700]to mitigate for your client's marginalizing therapy
- [00:03:30.460]and therapists fostering pride

- [00:03:35.050]The first theme discusses the idea that while TGD identity
- [00:03:37.980]and mental health can overlap,
- [00:03:39.850]there are nuances to this relationship
- [00:03:41.520]that therapists need to be able to navigate.
- [00:03:44.450]The second theme discussed the idea
- [00:03:45.910]that clients come into therapy
- [00:03:47.300]with the fear of being subjected to marginalizing behavior.
- [00:03:50.200]And it is up to the therapist to work with the client
- [00:03:52.390]to mitigate these fears.
- [00:03:54.700]The third theme discuss the idea
- [00:03:57.070]that it is necessary for therapists
- [00:03:58.880]to make it a point to be affirming
- [00:04:01.120]of clients' TGD identities
- [00:04:03.020]and understand how this identity played a role
- [00:04:05.010]in their daily life.
- [00:04:06.620]Also, encouraging them to be proud of this identity.
- [00:04:09.940]Ultimately, it was found that there was a link
- [00:04:11.810]between affirming behavior
- [00:04:13.090]and inclusive and effective therapy for TGD people.
- [00:04:16.460]The results showed that affirming videos
- [00:04:18.340]received the highest ratings
- [00:04:19.500]and least negative reactions from the participants.
- [00:04:22.340]It is important to note
- [00:04:23.490]that while our hypothesis was supported
- [00:04:25.360]and marginalizing behavior received the lowest ratings,
- [00:04:27.950]the neutral videos were overall also perceived as negative.
- [00:04:31.650]This allows for the assumption that neutrality
- [00:04:33.460]is not good enough when working with TGD clients.
- [00:04:36.350]This supports the idea that new therapeutic guidelines
- [00:04:38.660]for TGD people are necessary.
- [00:04:41.890]Some limitations of the study
- [00:04:43.310]were the geographical constraints.
- [00:04:45.420]Having done research using only participants
- [00:04:47.480]from the Central Great Plains
- [00:04:48.670]could affect the themes present.
- [00:04:50.970]Perhaps the future direction of this research
- [00:04:52.730]was to find participants from an environment
- [00:04:55.440]that has a larger TGD population
- [00:04:57.360]to see if the themes present would be different.
- [00:05:01.080]Another limitation of this population
- [00:05:03.080]was the racial and ethnic diversity.
- [00:05:07.200]Overall, most of the population examined was white,
- [00:05:10.890]which could have an impact on themes.
- [00:05:17.250]Ultimately, this study would like to bring light to the fact
- [00:05:19.640]that new therapeutic guidelines should be in place
- [00:05:23.500]that focus on making therapy more affirming
- [00:05:28.090]and effective for TGD clients.
